Review recurring operating issues each month
A monthly issue review looks for repeated causes rather than counting closed tickets. Group issues by room, asset, vendor or process and identify which changes could prevent recurrence. Keep the original incident records so a summary does not erase unresolved claims or owner approvals.
Evidence to assemble
Gather the source records before completing the worksheet. A missing record should remain an open question. Keep the original alongside any summary so the next person can trace the decision.

Work through the decision
- Group similar incidents.
- Distinguish repeated causes from unrelated events.
- Assign a corrective action with an owner.
- Review whether the action reduced recurrence.
For each action, name the person responsible for preparing the response and the person who approves it. Record what evidence would close the item. If new information changes cost, timing or scope, update the affected plan and identify who needs the revised version.
Common failure to avoid
A lower ticket count can reflect underreporting rather than better performance. Check the reporting process before treating the count as an improvement.
Worked example
A hypothetical property reports several missing remote controls. The review checks labeling, spare storage and turnover checks, assigns a change and reviews the next month’s incidents for evidence of improvement.
